Picture yourself standing on a ridge in the Everest region, the world's highest peaks painted gold by the sunrise. It's a moment you've dreamed of. But reaching that point safely requires understanding your body's greatest challenge at altitude: the thin air.

Here's the truth: Acute Mountain Sickness (AMS) doesn't care how fit you are or how much you've trained. It can affect anyone who ascends too quickly to high altitudes, whether you're a weekend hiker or an elite athlete. At elevations above 2,400 metres (8,000 feet), the air contains significantly less oxygen, and your body needs time to adapt.

This guide will walk you through everything you need to know about altitude sickness—what it is, how to spot it, and most importantly, how to avoid it. Because with the right knowledge and precautions, the Himalayas are not only achievable but unforgettable.


What Is Acute Mountain Sickness (AMS)?

Acute Mountain Sickness is your body's way of telling you it's struggling to cope with reduced oxygen levels at altitude. As you climb higher, atmospheric pressure decreases, meaning each breath delivers less oxygen to your brain and muscles.

Think of it as your body trying to run on less fuel than it's used to. Given time—usually a few days—most people acclimatise naturally. But if you push too fast, AMS can set in within 6 to 10 hours of arrival at a new altitude.

Who gets AMS? Anyone. Physical fitness doesn't protect you. Even Olympians can suffer. Living at sea level increases your risk. Previous altitude illness makes you more susceptible. The key factors are your rate of ascent, maximum altitude reached, and how high you sleep.


Common Symptoms: What to Watch For

Altitude sickness is a spectrum of severity. Here's what to look out for.

Mild AMS (The Warning Signs)

  • Headache – This is the key symptom. If you have a headache plus any of the following, it's likely AMS.

  • Nausea or loss of appetite – Food suddenly seems unappealing.

  • Fatigue and weakness – Even at rest, you feel drained.

  • Dizziness or lightheadedness – A feeling of being off-balance.

  • Insomnia – Difficulty sleeping, even when exhausted.

Key Reminder: Poor sleep alone is no longer considered a diagnostic symptom for AMS. It's the combination of a headache with one or more other symptoms that matters.

Symptoms typically last 24 to 48 hours and often feel like a hangover. Most people recover with rest and proper acclimatisation.

Severe AMS and Life-Threatening Conditions

If ignored, AMS can progress to life-threatening conditions:

High-Altitude Cerebral Edema (HACE):

  • Confusion and drowsiness

  • Difficulty walking (ataxia) – this is a crucial early warning sign

  • Altered mental state

  • Seizures and coma (in advanced stages)

High-Altitude Pulmonary Edema (HAPE):

  • Breathlessness even at rest

  • Persistent cough, possibly producing pink or bloody sputum

  • Extreme fatigue

  • Chest tightness

Critical: HAPE is the most common cause of death from altitude illness. Both HACE and HAPE require immediate descent. Delay can be fatal.


The Golden Rules for Prevention

Rule 1: Climb High, Sleep Low

This is the most effective acclimatisation strategy. The altitude at which you sleep matters more than how high you climb during the day.

The standard guideline: Above 3,000 metres, do not increase your sleeping altitude by more than 500 metres per day. Include a rest day (sleeping at the same altitude) every 3 to 4 days. On rest days, you can do day hikes to higher elevations, but always return to the lower altitude to sleep.

This strategy, often called "climb high, sleep low," gradually teaches your body to tolerate lower oxygen levels.

Rule 2: Pace Yourself

Slow and steady wins the race at altitude. Avoid overexertion, especially in the first 1 to 2 days after arrival. At high altitude, the air is so thin that just walking feels like a workout. Pushing too hard increases your risk of AMS.

Rule 3: Hydrate and Eat Well

Hydration: Aim for 3–4 litres of water daily. Dehydration can mimic AMS symptoms. Drink regularly, even if you don't feel thirsty.

Nutrition: Focus on high-carbohydrate foods. Carbohydrates are easier to digest at altitude and provide the energy your body needs to acclimatise. Local dishes like daal bhat are excellent choices—they're nutritious, carbohydrate-rich, and easy on the stomach.

Rule 4: Avoid Alcohol, Smoking, and Sleeping Pills

These substances impair your body's ability to acclimatise:

  • Alcohol and sedatives depress breathing, especially at night, and can worsen altitude symptoms.

  • Smoking reduces oxygen delivery to your tissues.

Rule 5: Consider Diamox (Acetazolamide)

Acetazolamide (commonly known as Diamox) is a medication that can reduce the likelihood of AMS and help treat symptoms if they develop. It works by stimulating more efficient breathing, helping your body acclimatise faster.

Important Disclaimers:

  • Diamox requires a prescription. Discuss it with your doctor before your trip.

  • It is not a substitute for proper acclimatisation. The recommended dose for prevention is typically 125 mg twice daily, starting the night before ascent.

  • If you have a sulfonamide allergy, consult your doctor; a supervised trial may be recommended.

Other Medications: Dexamethasone is another option for treating AMS, but it does not aid acclimatisation—it masks symptoms. NSAIDs like ibuprofen can help prevent or treat high-altitude headache.


The Ultimate Rule: When to Descend

If symptoms get worse or do not improve with rest, the only cure is to descend.

This is the most critical rule of high-altitude safety. Descent of 300 to 500 metres is often rapidly effective for AMS. For severe symptoms or any signs of HACE or HAPE, immediate descent (and helicopter evacuation if available) can be life-saving.

Do not wait to see if things get better. Don't continue climbing in the hope that you'll "push through it." Listen to your body and respect the altitude.
